Wisdom Tooth Removal: Recovery Time, Aftercare and What It Really Feels Like

14th September 2026

Most people recovering from wisdom tooth removal feel noticeably better by day four and close to normal within one to two weeks. The worst of it is usually days two and three, and it is swelling and jaw stiffness rather than sharp pain that people find hardest.

That is the honest headline. What follows is what happens on the day, how recovery unfolds, the aftercare that changes your outcome, and the warning signs that mean you should pick up the phone. If you are weighing up whether to go ahead at all, our oral surgery explains how we assess each case.

Do You Actually Need Your Wisdom Teeth Out?

This question deserves asking, because the answer is often no. Guidance from the National Institute for Health and Care Excellence is clear that removing healthy impacted wisdom teeth as a precaution should not happen routinely on the NHS. Extraction is for teeth showing evidence of a problem.

Recognised reasons include decay that cannot be restored, repeated infection of the gum over a partly erupted tooth, abscess or cellulitis, cysts, damage to the neighbouring molar, or a tooth obstructing other planned surgery. The full NICE guidance on the extraction of wisdom teeth has the detail.

One nuance worth knowing. A tooth that surfaces only partially leaves a hood of gum above it, which readily becomes swollen and infected. Dentists call this pericoronitis. It is common, and one mild episode is not by itself grounds for removal. Repeated episodes usually are. If your gums are sore and bleeding more generally, that is a separate issue covered on our gum disease treatment.

What Happens on the Day

Simple Extraction or Surgical Removal

If the tooth has erupted fully and sits in a straightforward position, it can often be lifted out much like any other. Upper wisdom teeth usually fall into this group and recovery is quick.

Impacted lower wisdom teeth are a different job. Gum is eased back, a little surrounding bone may be trimmed, and the tooth is frequently divided so it can be lifted out in sections through a smaller opening. Dissolvable stitches finish the job. Recovery is more noticeable, reflecting the work.

Assessing the Nerve Before Anything Starts

Feeling in the lower lip and chin travels along the inferior alveolar nerve, which runs through a canal in the jawbone that a lower wisdom tooth root may press against. Where that happens, some patients notice altered sensation afterwards, and for most it fades within weeks. Permanent change is rare, under one in a hundred.

This is why imaging earns its keep. An ordinary radiograph flattens everything into two dimensions, whereas a three dimensional scan reveals the true relationship between root tips and canal. When they prove intimate, the plan changes. One option is a coronectomy, taking only the upper portion of the tooth and deliberately leaving the root section in place. We have an on site CBCT and OPG imaging centre, so it is done in house rather than after a referral and a wait.

What You Will Feel

With local anaesthetic you will feel firm pressure and some noise, but not pain. Patients are often surprised the pressure is the strange part rather than any discomfort. If the idea makes you tense, say so in advance. Sedation dentistry is available for longer procedures and anxious patients, and our guide to overcoming dental fear covers the practical side.

Wisdom Tooth Removal Recovery Time

Recovery follows a predictable shape. Surgical removal of a lower impacted tooth sits at the longer end, a simple upper extraction at the shorter end.

Stage What most people experience
First 24 hours Some oozing, numbness wearing off, mild to moderate ache. Rest.
Days 2 to 3 Swelling and jaw stiffness peak. This is the low point.
Days 4 to 7 Swelling settles, bruising may appear then fade, eating gets easier.
Days 7 to 14 Stitches dissolve, most people feel normal, socket still closing.
Weeks 3 to 8 Bone quietly fills the socket. No symptoms expected.

The First 24 Hours

Keep light pressure on the gauze pad for roughly an hour. A little oozing afterwards is expected. Stay upright, rest, and hold an ice pack to the cheek in short bursts. Otherwise leave the area alone: no rinsing, no spitting, nothing hot to drink. A clot is forming in the socket and that clot does the healing.

Days Two and Three

This is the peak. Puffiness is at its greatest, the jaw muscles tighten and you may struggle to open wide. Any bruising along the jawline looks far more alarming than it is. After the first day you can begin warm salt water rinses, letting the water sit and drop out rather than swilling it around.

Days Four Onwards

Things turn a corner. Swelling reduces, eating gets easier and most people doing desk based work are back to it. Keep exercise light for a week, since raising your heart rate early is associated with clot problems. Stitches dissolve within one to two weeks. The socket takes several weeks to fill with bone, but you will not feel that.

Aftercare That Genuinely Makes a Difference

  • Hold off smoking and vaping as long as you can, a week if possible. Nothing else within your control has the same influence on how comfortable recovery is.
  • Skip straws entirely, and do not spit or rinse hard on day one. Anything generating suction in the mouth works against the clot.
  • Get ahead of the discomfort by taking pain relief while the anaesthetic is still working, rather than waiting until it bites. Leave aspirin alone, as it interferes with clotting.
  • Stick to soft food that is not hot. Rice, seeds, nuts and crisps work into the socket and are miserable to get back out.
  • Carry on brushing the rest of your teeth as usual. Healing is quicker in a clean mouth, so steer around the surgical site only, and only for a day or two.
  • Had an upper wisdom tooth taken out? Go gently when clearing your nose for the next week.

The NHS guide to wisdom tooth removal also covers when it is safe to drive again, which depends on whether you had local anaesthetic, sedation or a general anaesthetic.

Dry Socket and How to Avoid It

Dry socket is what occurs if that protective clot disappears before the gum has closed over the site, so the bone beneath it sits uncovered. It is the complication patients ask about most, and it is very largely within your control.

Timing is what gives it away. Normal soreness eases steadily once you are past the third day. Dry socket reverses that pattern, with discomfort that had been improving turning sharply worse in the middle of the first week. Patients describe a deep, relentless throb travelling towards the ear, often with an unpleasant taste. Risk climbs if you smoke, use a straw too soon, or if the tooth was an awkward lower one.

None of this is dangerous and it responds well to treatment. Irrigating the socket and placing a medicated dressing usually settles things quickly. What you should not do is wait it out at home hoping it passes.

When to Contact a Dentist

Some discomfort is expected. These are not.

  • Pain that increases after day three rather than easing.
  • Bleeding that will not stop after twenty minutes of firm pressure on fresh gauze.
  • Spreading facial swelling, difficulty swallowing or breathing, or a fever. Swelling tracking towards the eye or down the neck needs same day attention.
  • Numbness in the lip, chin or tongue still present several days later.
  • Being unable to open your mouth more than a couple of centimetres after a week.

We keep appointments free for urgent problems, and our article on when a tooth problem warrants A and E explains which cases belong in hospital.

What It Costs

Unlike cosmetic work, wisdom tooth removal is clinically necessary and available on the NHS. It falls under Band 2, covering extractions, fillings and root canal treatment, and you pay one band charge for the whole course however many appointments it takes. Charges are reviewed each April, and current NHS and private fees are on our fees.

Privately, simple extractions start lower and surgical removal of an impacted lower tooth costs more, reflecting time, imaging and complexity. We accept accept referrals from other practices too.

Frequently Asked Questions

How Long Does Wisdom Tooth Removal Take?

A simple extraction takes a few minutes. Surgical removal of an impacted lower wisdom tooth usually takes twenty to forty minutes, plus time for anaesthetic and aftercare instructions.

How Long Should I Take Off Work?

A day or two is typical for desk based work. Physical jobs need longer, closer to a week, because exertion raises the risk of disturbing the clot.

Can I Have All Four Wisdom Teeth Removed at Once?

Yes, and some prefer one recovery period to four. It does mean a rougher few days and a fully soft diet. Others do one side at a time so they can still chew.

When Can I Eat Normally Again?

Soft food for the first few days, then build back up as comfort allows. Most people eat normally within a week, though keep hard food away from a lower socket a little longer.

Does Wisdom Tooth Removal Hurt Afterwards?

Expect an ache rather than sharp pain, well managed by over the counter pain relief. Most find swelling and jaw stiffness more limiting than the pain.

Will Removing My Wisdom Teeth Straighten My Other Teeth?

No. This is a persistent myth. Crowding of the lower front teeth happens with age whether or not wisdom teeth are present, and removal is not a treatment for it.

What if My Wisdom Tooth Is Not Causing Any Problems?

Then leaving it alone and monitoring is usually the right call, in line with national guidance. Your dentist should keep an eye on it at routine check ups.

Talk It Through Before You Decide

Wisdom teeth are an area where patients are often told something should come out without being told why. A proper assessment explains what the tooth is doing, whether it needs removing at all, what the imaging shows about the nerve, and what recovery realistically looks like.

Lakewood Dental is on Middleton Road in Carshalton, minutes from Sutton, Wallington, Cheam, Rosehill and Morden. We see NHS and private patients, have CBCT and OPG imaging on site, and offer sedation for anyone who finds treatment difficult. You can read about the clinicians who would be caring for you.

Call us on 020 8648 3888 or book an assessment online and get a straight answer about whether your wisdom teeth need anything doing at all.
