Jaw Clicking: What Causes It and When It Points to TMJ

22nd September 2026

That small click when you bite into a sandwich or yawn tends to catch people off guard the first time they notice it. Jaw clicking is one of the most frequent concerns patients raise with us at Lakewood Dental in Carshalton, and the reassuring part is that a click on its own is rarely serious. What matters far more is the company it keeps.

A painless click that has been there for years usually needs nothing done about it. A click arriving alongside aching, stiffness or a jaw that catches as you open is a different conversation. This guide explains what produces the sound, which causes are worth investigating, and the signs that suggest a temporomandibular disorder rather than a harmless quirk of how your jaw moves.

What Is Actually Happening When Your Jaw Clicks

Your lower jaw meets the skull at two joints, one sitting just in front of each ear. Rest a fingertip there, open slowly, and you will feel bone glide forward and downward beneath the skin. Unlike a knee or an elbow, this joint has to do two jobs at once. It rotates, and it slides. Managing both movements is a small pad of dense fibrous tissue that rides on top of the bone and should travel forward with it, staying sandwiched between the two bony surfaces throughout.

Jaw clicking happens when that pad stops keeping pace. If it slips slightly ahead of the bone while your mouth is shut, the bone has to catch up and snap back underneath it as you open. That moment of recapture is the noise you hear, and many people get a second, quieter click on the way back down.

Clicking Is Not the Same as Grating

There is a meaningful difference between a click and a gravelly noise. A crisp click or pop usually points to the disc moving out of sequence. A rougher, sandy sound, which dentists call crepitus, suggests the joint surfaces themselves have worn. Our routine dental check-ups include an assessment of how far and how evenly your jaw moves, so changes get picked up early rather than years down the line.

Common Causes of Jaw Clicking

Grinding and Clenching

Bruxism, the habit of grinding or clenching your teeth, is the cause we see most often. It rarely happens consciously. Most people do it while asleep or while concentrating hard, and the first real evidence tends to be flattened, shortened teeth rather than any memory of doing it. Clenching loads the joint and fatigues the muscles working it, which makes existing clicking louder and more frequent.

Worth being precise: grinding does not directly push a disc out of position. It aggravates a joint that is already struggling and wears down the teeth holding your bite at the right height. Where we spot those signs, tooth wear treatment and management tackles both the damage and the habit driving it.

A Bite That Has Shifted

Your teeth and your jaw joints work as a single system. When the bite changes, the joints adapt, and not always comfortably. Gaps left by lost teeth are a frequent culprit, because neighbouring teeth drift into the space and the opposing tooth drops down, altering how the jaws meet. New crowns, fillings or dentures sitting slightly high do the same. If you have been living with spaces, replacing missing teeth often settles jaw symptoms as a welcome side effect of restoring the bite.

Injury, Strain and Everyday Habits

A knock to the chin can displace the disc outright, and so can holding your mouth wide open for a long stretch. Day to day, the usual suspects are hours of chewing gum, biting pens, propping your chin on your hand at a desk, and wedging a phone between shoulder and ear.

Arthritis affects this joint as it does any other, and rheumatoid arthritis, gout and fibromyalgia are all associated with jaw symptoms. Stress belongs on the list too, not because the pain is imagined, but because tension reliably translates into clenched muscles.

When Jaw Clicking Points to TMJ Disorder

Temporomandibular disorder, or TMD, is the umbrella term for problems affecting the jaw joints and the muscles that operate them. NHS figures put it at roughly one in fifteen people in the UK, which makes it considerably more common than most patients expect.

The distinction that matters: clicking by itself, with no pain and no restriction, is not a disorder. The US National Institute of Dental and Craniofacial Research states plainly that painless joint noises are considered normal and need no treatment. TMD becomes worth suspecting when the click starts travelling with other symptoms.

Signs That Warrant an Appointment

  • Pain around the jaw, ear or temple, often worse while chewing
  • A jaw that catches, sticks or locks, either open or closed
  • Difficulty opening your mouth as wide as you once could
  • Headaches concentrated around the temples
  • Earache, or a blocked or buzzing sensation with no infection present
  • A bite that has suddenly started to feel uneven or unfamiliar

Locking deserves particular attention. If your jaw stops part way open and refuses to move, or you wake unable to open more than a couple of centimetres, the disc has most likely jammed rather than recaptured. Get that seen promptly, because a locked joint becomes harder to treat the longer it stays that way.

Separately, the NHS lists symptoms needing same day attention, including being unable to eat or drink, frequent severe headaches, scalp tenderness, and any change to your vision. Those point to conditions beyond the jaw joint and should not be left.

How the Cause Is Identified

Diagnosis starts with listening and looking, not scanning. We will ask when you first noticed the jaw clicking, whether it has changed, and how your sleep and stress levels have been. Then comes the examination: how far and how symmetrically your jaw opens, whether the joints and muscles are tender, and what wear patterns your teeth reveal about grinding.

Imaging is not needed in every case, but where a joint problem is suspected rather than a muscular one, detailed views help. Our on site CBCT and OPG dental imaging centre produces three dimensional scans of the jaw joints in house, so you are not sent elsewhere and left waiting weeks. Should your case need specialist input, there is an experienced oral surgeon among our clinical team, which keeps any referral straightforward.

What Actually Helps

Changes Worth Making Now

Most temporomandibular symptoms settle with conservative measures, and the large majority of cases are managed in general practice without surgery entering the conversation. Start here:

  • Rest the joint. Soft meals for a fortnight, cut small, and nothing needing real force to chew.
  • Apply warmth or cold to the side of the face, whichever gives more relief, wrapped in a cloth.
  • Massage the muscles just in front of and below the ear using slow circles.
  • Stop testing it. Opening repeatedly to check the click is still there keeps the joint irritated.
  • Support your jaw with a hand when you yawn, and drop the chewing gum, pen chewing and nail biting.
  • Watch your resting position. Teeth should sit slightly apart with lips together, never clamped.

Splints and Night Guards

Where grinding is driving the problem, a custom made splint worn overnight separates the teeth, spreads the load and stops the muscles generating the force that does the damage. Shop bought guards tend to be a false economy, because one that fits poorly can shift your bite and leave you worse off. A splint built from impressions of your own teeth fits precisely and lasts.

Rebuilding Teeth That Have Worn Down

Years of grinding shorten teeth and flatten the surfaces that guide your bite. Restoring proper height and shape often calms the joint alongside it. Depending on how much natural tooth remains, that might mean onlays and overlays where a partial restoration does the job, or dental crowns where more support is needed.

If dental visits make you anxious, which is common with jaw problems given the examination involves the sore area itself, we offer sedation for nervous patients so getting assessed does not become something you keep putting off.

Frequently Asked Questions

Should I Worry About Jaw Clicking With No Pain?

No. A painless click that does not limit how wide you can open is common and generally needs no treatment. Mention it at your next check-up so it goes on record, and seek advice if pain, locking or restricted opening develops later.

Can Jaw Clicking Go Away on Its Own?

Often, yes. Most temporomandibular symptoms improve within a few months with rest and self care. They tend to come and go rather than resolve in a straight line, flaring during stressful spells or when you are sleeping badly.

Does Jaw Clicking Mean I Have TMJ?

Not by itself. Everyone has temporomandibular joints, so the phrase is misleading. What people mean is temporomandibular disorder, and that needs pain, restricted movement or dysfunction alongside the noise. Clicking alone does not qualify.

Why Does My Jaw Click on One Side Only?

Single sided clicking is common and usually reflects the disc on that side slipping out of sequence. Chewing habitually on one side, an uneven bite, or an old injury can each explain the asymmetry.

Can a Dentist Treat Jaw Clicking?

Yes, and a dentist is usually the right first port of call. We can tell jaw joint pain apart from toothache and assess the bite and wear patterns behind it. Treatment normally involves advice, a splint and bite corrections, with specialist referral needed only rarely.

Does Jaw Clicking Cause Headaches?

It can contribute. The muscles that move your jaw extend up into the temples, so when clenching overworks them, headaches around the sides of the head are a frequent result.

A Note on Medical Advice

This article is for general information only and does not replace a clinical assessment. Jaw symptoms vary from one person to the next, and advice that suits one patient may not suit another. Please speak to a dentist or your GP about your own circumstances.
